FAA航空身体検査を受験するに当たって

FAA Aeromedical Examinations

Please read carefully before you apply the FAA aeromedical examinations.
FAA(米国連邦航空局)指定の航空身体検査を受験するに当たって、以下の事項にご注意下さい。

■Applying the MedXpress 申請手続き

●Access to MedXpress（http://medxpress.faa.gov/ ）before you book to our clinic. You can’t take any class medical exams without your confirmation number.
ｲﾝﾀｰﾈｯﾄでMedXpress（http://medxpress.faa.gov/）へｱｸｾｽし、航空身体検査の申請を完了させて下さい。完了していないと、どのｸﾗｽの身体検査も受けられません。

●The confirmation number issued by MedXpress will be expired in 3 months. You need to reapply when you take the exam 3 months later or more.
MedXpressが発給したconfirmation number(受験番号)の有効期間は3か月間です。それ以降に受験する場合は、再度申請し直す必要があります。

●Please do not mistype your first name and last name. Your application name will appear automatically on your FAA medical.
First name(名)、Last name(姓)を間違いなく記入して下さい。逆にすると、そのまま航空身体検査証明に記載されてしまいます。

●Please select the class you take. If you won’t select any class, you’d apply the medical exam for air traffic controllers. Here you can take the 1st, 2nd and 3rd class airmen’s medical examinations only.
1st class(定期航空事業用操縦士に相当)、2nd class(事業用操縦士に相当)、3rd class(自家用操縦士に相当)のどれを受験するか、忘れずに選択して下さい。無記入の場合は、自動的に航空管制官用の身体検査となり、当院では受験できなくなります。

●Please inform your Social Security Number (SSN), but you can choose “not inform”. If you don’t have your SSN, MedXpress would automatically assign a new number.
ｿｰｼｬﾙｾｷｭﾘﾃｨｰﾅﾝﾊﾞｰ(SSN、社会保障番号、日本のﾏｲﾅﾝﾊﾞｰに相当)をお持ちの方はそれを記入するか、SSNは通知しないとして下さい。SSNを持たず、無記入の場合は、MedXpressから自動的にSSNが発給されます。

●If you received the FAA medical before, please check the date of examination on your aeromedical certificate.
以前にFAA航空身体検査証明を発給されたことがある場合は、その発給年月日を調べておいて頂きます。

●FAA doesn’t allow to wear contact lens on one eye. One contact lens for distant vision correction and the other lens for near vision would not be accepted. If you choose “Yes” to the question 17b (Do you ever use near vision contact lens(es) while flying?), you need to prepare the contact lens　reflection format prescribed by ophthalmologist. We’ll check your near and distant vision acuity without contact lenses also.
FAAでは、ｺﾝﾀｸﾄﾚﾝｽﾞを片眼だけ使用して操縦することを認めておりません。片方が遠見用、もう片方が近見用の場合も操縦不可です。「飛行中に、近見用ｺﾝﾀｸﾄﾚﾝｽﾞを使用しているか？」の問17bに”Yes”を選択する場合は、両眼のｺﾝﾀｸﾄﾚﾝｽﾞの度数を事前に眼科で調べておいて頂かないと、当日受験できません。当日は遠見と近見の裸眼視力も航空身体検査で調べますから、ご注意下さい。

●If you take any medications, please input generic name (not commercial brand) with daily dose.
常用薬剤があれば、処方された薬局に連絡して、英語の一般名を確認して、1日服用量を記入して下さい。

●Regarding your medical history, national driver register and certifying declaration (item20), we’d advise to inform honestly. If unreported violations or penalties were found, FAA might deny all relevant certificates you hold.
ご自身の病歴、米国における交通違反や犯罪歴(item20)等については、正直に申告することをお勧めします。後日発覚した際には、FAAが管轄する全ての資格を剥奪される恐れがあります。

●After the all information input in MedXpress, your confirmation number will be indicated on the bottom of the sheet. We’d recommend to print the sheet and bring to the clinic.
全て記入すると、MedXpressがconfirmation number(受験番号)を表示してきます。画面をﾌﾟﾘﾝﾄｱｳﾄし、受験当日に持参することをお勧めします。

●Please book the date and time of your exam with your confirmation number (TEL.044-969-3001). Upon the exam, your health conditions should be very good and you should expect 1-2 hours for exam and issuing process.
受験番号が決まったら、電話予約を取ります(TEL.044-969-3001)。体調がすこぶる良好で、1‐2時間ほど時間の余裕が取れる日時にしましょう。

■Receiving the exam 航空身体検査の受検

●Applicants are strongly advised to be the best health conditions. Drinking opportunities, night work shift or longtime flight or driving previous day would compromise your health checkup in blood pressure, visual acuity, urine test or ECG. Principally, your exam results might be referred to FAA, even if one abnormality. Once referred, it takes at least 2-3 months to receive the FAA decision.
万全の体調で受験するよう、心がけて下さい。前日に飲み会、夜勤のﾊﾞｲﾄ、長距離の飛行・運転をして受験すると、血圧、視力、尿検査、心電図検査等で思わぬ不覚を取ることがあります。原則として、検査項目が1つでも不適合の場合、FAA本部の指示による再検査となってしまいます。そうなるとFAA本部からの航空身体検査証明の発給は、最短でも2-3か月先となります。

●Your daily use glasses may not be best fit. Especially young applicants and student cadet often compromise visual acuities in a short period of time. To ensure that your visual acuities, both distant and near, would be 20/20 or better, we’d advise to check at ophthalmologist or lens shops in advance.
普段着用している眼鏡を持って来ても、屈折度数があっていないと必要な視力が出ないことがあります。特に大学生の受験者で、短期間に視力が悪化している事例を散見します。念のため、眼鏡店や眼科の外来で、視力が1.0超出ていることを確認してから受験することをお勧めいたします。

●FAA requests the examiners to identify the applicant using photo IDs (e.g. passport, drivers’ license, student ID, employee ID, etc.) before starting the exam.
受験に当たり、FAAは顔写真付き証明書で本人確認を求めております。適当な書類(旅券、ﾏｲﾅﾝﾊﾞｰｶｰﾄﾞ、運転免許証、学生証、社員証など)を持参下さい。

●The exam fee will be 21,000 JPY, regardless the class you take. We won’t accept credit cards. In case of additional tests, if necessary, health insurance documents would be accepted.
当院での受験料は21,000円です。ｸﾚｼﾞｯﾄｶｰﾄﾞ払いが出来ませんから、ご注意下さい。なお追加検査が必要な場合は、健康保険を適用できますから、念のため健康保険証を持参されることをお勧めいたします。

●If applicants have been treated or under a follow up period of some diseases, please bring the recent clinical data. The aviation medical examiners may be allowed to issue the aeromedical certificate immediately without referral to FAA .
現在治療中の病気、治療後の経過観察期間中の疾病がある場合は、最近の臨床ﾃﾞｰﾀをご持参下さい。病気の種類や病状の程度によっては、FAA本部へ照会しなくても、即日発給する事が出来る場合があります。
